I know it can be frustrating!
I'm attaching a sample with all the fields. You can use that as a template if it helps. Just remember, you'll need to save it as a text file before you upload it.
If you click on Catalog--Import/Export, there is a sample file and instructions on what to put in each field for your particular products.
It's a rare to get it right on the first attempt! Let us know how it goes and if you run into problems. Someone will help you through it.

If you are using Excel, go to file, save as, at the bottom of the spreadsheet it will ask you to name the file, in the next box down is ask for type, click on the drop down box and you should find a line saying Text ( tab delineated). Click on that and then click save.
If you are using Microsoft works, it goes the same way except in the drop down box, if I remember right it just says text file, click and save.

I use open office spread sheet for mine. I just copy and paste all the columns and rows at one time into notepad. Notepad will automatically save it as a .txt file. That file will upload.

you have empty rows between the column headings and the first product.
